Booking Process: Counselling Services and Play Therapy

Please complete the Intake Referral Form​

• ​This can be completed by parents, caregivers, ​specialists or other allied health providers.

We will contact you once we have received and processed the referral to proceed with booking your initial consultation.

Our referral processing times is approximately 5-7 business days.

• If you do not hear back from us after this time, please do not hesitate to contact us to follow up with your referral.​

We will provide you with an introduction letter confirming your initial appointment dates and times and our patient registration and consent form.

1 week before consultation: 

• You will receive an SMS confirmation for your appointment booking and a payment link to complete $100 deposit payment. Appointment will only be confirmed once deposit payment is received.

• If you are unable to attend the appointment, please call us at least 2 full working days in advance to avoid late notice fee.

• If we do not receive a reply and are not able to process a payment hold within 5 days, your booking will be cancelled.

Complete the patient registration and consent form

Important documents:

• ​NDIS Plan / Medicare / Private health fund details

• Letters / reports: doctors, psychologist, school, NDIS providers etc.
