The approval of this request is at the Psychiatrist's discretion.
Once the request is approved, you will be sent an e-pay link to complete payment ($50.00). After payment is received, your e-prescription will be sent via SMS.
The minimum processing time is 5-7 business days.
If you require an urgent prescription, please visit your GP to request for this.
PLEASE NOTE: If you have not seen your treating Psychiatrist for more than 6 months, please do not complete this form - contact us to book a consultation for a medication review.
